According to the Dynamics of Nursing: Art and Science of Professional Practice, there are eight principles within healthcare ethics. Autonomy, beneficence, fidelity, justice, non maleficence, privacy, confidentiality, respect, and veracity.  In this scenario I feel that Veracity would be the most appropriate ethical principle in this situation.  Veracity means truthfulness and honesty, and a truthful disclosure of information.  Critical thinking and judgment also play a part in your decision making and approach to this situation.  You need to identify the situation at hand.  From there it is important to utilize your own personal beliefs and values, nursing knowledge, ethical reasoning, and intuition.  From there the nurse can respond by using the critical thinking and analyzed reasoning to make her approach to the physician.   After having the interaction with the physician and based on how the physician responded to the situation, reflection is the last piece the nurse would do to reflect and react based on the outcomes of the physician.

I would approach this scenario in the following steps using the ethical principle of veracity as my base.

Step 1: I would approach the physician in private and explain the discrepancy and ask for clarification.

Step 2: If there was still a discrepancy in what was in the chart and what the physician stated during patient rounds, I would advocate on behalf of the patient.  If the physician refuses to agree or change the discrepancy I would bring it up the chain of command and alert my supervisor.

Step 3:  I would make note of the discrepancy with my supervisor and correctly chart the patient’s level of consciousnesses in my assessment.
